Triathlon entry forms Minimize

IMPORTANT INFORMATION

 
  • All competitors must register before 9.30am 13th February 2010
  • Registration will take place from 8.30am - 9.30am at Lake Centenary Temora
  • A race brief will be held 15 minutes prior to the race
  • Entry fee will be paid on the day of the event.
  • Could you please fill in the nomination form in advance so the the event organisers get an idea of potential competitor numbers, this will help in the organisation of the race.
  • A Bus will running from Nixon Park back to Lake Centenary from 11.30 am onwards.
  • For non Triathlon Australia members there will be a 1 day insurance fee of $4.50 per individual & $9.00 per team.

Participants Agreement must be signed by all entrants/competitors.

I agree to participate in the Temora Golden Gift Triathlon, which is being held on 13th February 2010 on the following basis:

By participating I accept all risks necessarily flowing from my participation, which would result in loss of life or any injury loss or damage. I release all people associated with the conduct of the Triathlon from, and will indemnify them against, all liability (including liability for their negligence) for all injury, loss or damage arising out of or connected with my participation in the Triathlon. For clarification, the people released include event organizers, promoters, sponsors, managers, government and public authorities, volunteers, and medical personnel. This release and indemnity continues forever and binds heirs, executors, personal representatives and assigns.

I acknowledge the Triathlon involves the real risk of serious injury, or risk of death from various causes. I understand that I should not participate in the Triathlon unless I have prepared appropriately and am medically fit.

I certify that I am 18 years of age or older and I have read this document and fully understand it.

IMPORTANT Any competitor under the age of 18 years requires Declaration by Parent or Guardian as parent Guardian of the entrant/competitor. I agree the above for myself and on behalf of the entrant/competitor.
